The PE Global Care Connect Rapid Response Team specialises in providing essential emergency support for individuals experiencing crises.

Our main objective is to prevent unnecessary hospital admissions, secure appropriate unit placements, and avoid service disruptions that can lead to unsuitable care environments, bed shortages, and increased security unit escalations.

At PE Global Care Connect Rapid Response Team, we are dedicated to delivering a dependable solution that prioritises the needs of service users, ensuring uninterrupted care. Our team consists of highly trained professionals experienced in assisting individuals with physical disabilities , dementia care, Complex Behaviours and needs and Mental Health issues.

All staff utilise a Positive Behaviour Support (PBS) approach, applying their extensive expertise to deliver the best possible care.

Available 24/7, our service is always prepared to address any situation as it arises, providing unwavering support to ensure the ongoing well-being of those in crisis.

We collaborate closely with various organisations, including Integrated Care Boards (ICBs) and Local Authorities (Councils), to support emergency  healthcare needs and work directly with the MDT, We also partner with Adult Mental Health Services and Social Care teams, promoting a comprehensive support strategy that encompasses home, supported living communities and community.

Our commitment extends beyond merely managing crises; we aim to create lasting solutions that empower individuals, foster independence, and enhance holistic well-being. Empathy is at the core of our approach, as our specialised nurses and healthcare assistants provide outstanding care to young adults , and adults in difficult situations.
